Baek Kang-Hyuk (Ju Ji-Hoon) is a genius trauma surgeon who has performed surgeries in conflict zones around the world. He is confident in his abilities and has a bulldozer-like personality when he thinks he is doing the right thing. He begins to work at a university hospital and leads the severe trauma team there. The severe trauma team is a double-edged sword for the university hospital. This is because as the severe trauma team saves more patients, the hospital goes further into the red financially. Besides Baek Kang-Hyuk, the severe trauma team consists of Yang Jae-Won (Choo Yeong-Woo), Nurse Cheon Jang-Mi (Ha-Young), and anesthesiologist resident Park Gyeong-Won (Jeong Jae-Kwang). Yang Jae-Won belonged to the proctological surgery department led by Professor Han Yu-Rim (Yoon Kyung-Ho), but Yang Jae-Won was scouted by Baek Kang-Hyuk and joined his team.
原负责韩国大学外伤重症小组的教授过劳病倒，通过保健福祉部部长姜明熙（金善映 饰）的推荐，来自三流医学院的白江赫（朱智勋 饰）空降来袭。他专注把人救活，只要遇上阻碍直接开怼，对医术近乎苛求的白医生，扬言要改善因非专业、判断时间过长、加上人力不足及预算短缺、导致原本救得活的病人错失黄金时间而死亡的情况，将没人要应征的小组扩大成外伤重症中心；第一个动作就是把原本是肛门外科的学霸杨载源（秋英宇 饰）骗过来，因为每救一个病人，就花费高昂，所以医院的院长崔朝殷（金义城 饰）正虎视眈眈要以亏损之名大肆砍掉他的预算。
